You are married and want your spouse's name added to your passport records — commonly asked for alongside a reissue application or a change-of-particulars application, as Annexure J of the Passport Seva instruction booklet. Confirm against the current instruction booklet or with your Regional Passport Office that Annexure J is still the letter used for this before relying on this template's title — annexure lettering has been revised across editions of that booklet. Not for changing your own name after marriage — use the Passport Annexure Affidavit (Annexure F) for that instead. Not for removing a former spouse's name after divorce, which is usually a different declaration this template does not cover.
What follows is written for Punjab and Chandigarh. It states the position there rather than describing how it varies, so the amounts below are the ones that apply — worth confirming at the sub-registrar’s office, because rates change. If the property is anywhere else in India, none of this is yours.
Stamp duty
A general affidavit, chargeable under Article 4 (Affidavit) of Schedule 1-A of the Indian Stamp Act, 1899 as applicable to Punjab, and under the corresponding entry of that Schedule as applicable to the Union Territory of Chandigarh — commonly a small fixed value, typically Rs 10, on non-judicial stamp paper or an e-stamp certificate. Confirm the current value with the stamp vendor, and separately confirm with the Passport Seva Kendra whether it expects a particular stamp value or paper for this affidavit.
Registration
Not compulsorily registrable. This affidavit is submitted with the passport application and kept on the Passport Seva file; it does not go to the Sub-Registrar.
Notarisation
Sworn or affirmed before an attesting officer of a kind the Passport Seva instruction booklet currently accepts — ordinarily a Notary Public appointed under the Notaries Act, 1952, or an Executive Magistrate. Confirm the currently accepted list before the appointment, and sign only in the attesting officer's presence.
Witnesses
Not compulsory for the affidavit itself.
