When you need it

A sworn statement for the school. You bring up a child alone, or you are the child's guardian, and the school wants it on oath: who the child lives with, why only one parent or guardian signs the school's forms, and that the school may deal with you alone.

It is commonly asked for at admission, and again when a form needs both parents' signatures and there is only one. It records the facts and what you undertake: to sign the school's forms, to answer for the child's fees and conduct, and to tell the school if the position changes. It records why the other parent is not involved, whether that parent has died, the parents are divorced or apart, or the other parent cannot be traced. Attach the paper that proves it, such as a death certificate or the decree of divorce or the court order on custody. It does not give you custody, take away the other parent's rights, or settle who the child lives with: that is for the parents' agreement or a court. Not for the child's travel abroad: use the Parents' Consent for Minor Travelling Abroad. Not an agreement on custody: use the Child Custody Agreement.

AFFIDAVIT OF A SINGLE PARENT / GUARDIAN

I, Rajwinder Kaur, son/daughter/wife of Sh. Gurnam Singh, aged 36 years, resident of House No. 207, Sector 70, Mohali 160071, do solemnly affirm and state on oath as follows:

1. That I am the mother of Aarav Singh, born on 1 April 2026 (the child).

2. That I make this affidavit for St. Xavier's School, Mohali (the school), for admission to Class 6 for the session 2027-28.

3. That I am the only parent or guardian who deals with the school on the child's behalf, because the other parent of the child has died.

4. That the details are: The child's father died on 3 March 2022; his death certificate is attached.

5. That the child lives with me and I bring up the child and meet the child's expenses, including the school fees.

6. That I will sign the school's forms and declarations for the child, answer for the child's fees and conduct, and be the person the school contacts about the child.

7. That I will tell the school at once if the position stated above changes, including if the child's other parent returns to the child's life or any court passes an order about the child's custody.

8. That this affidavit is made so that the school may rely on it for the purpose stated above, and it is not an order of custody.

VERIFICATION

Verified at Mohali on 2 April 2026 that the contents of this affidavit are true and correct to my knowledge, that no part of it is false and that nothing material has been concealed.
