AFFIDAVIT OF SURNAME AND GIVEN NAME
I, Gurpreet Singh, S/o Sh. Harbans Singh, resident of House No. 88, Sector 40-B, Chandigarh, holding passport number N4567890, do hereby solemnly affirm and declare as under:
- 1. That my passport shows "Singh" in the surname box and "Gurpreet" in the given-name box.
- 2. That in the visa and immigration forms I am filling in for Canada, I will enter "Gurpreet" as my given name and "Singh" as my surname.
- 3. That this is the way my name has come to be written, namely: My name was recorded as Gurpreet Singh at birth. I have never used any other family name, and the passport office printed it in the surname box only.
- 4. That the records in which my name appears read as follows: Aadhaar: Gurpreet Singh; PAN: Gurpreet Singh; Class X certificate: Gurpreet Singh; degree: Gurpreet Singh
- 5. That the person named in all those records and in my passport is one and the same person, namely myself, and that I have not used or held any other name.
- 6. That this affidavit is sworn to be produced before the visa or immigration authority of Canada, or any other authority that may require it, and may be relied upon by that authority.
