PAN CR-01: request for changes or correction in PAN data (individual)

PAN CR-01 is Protean's request for changes or correction in PAN data for an individual. Use it when something printed on your existing PAN is wrong or has changed. The form has a tick box in the left margin of each item, and you tick only the items you want changed. The guidelines on the form say to attach a copy of your PAN card (or an FIR copy if it is lost) and the proofs for each change.

Issued by: Income Tax Department, Government of India (application form under rule 158 of the Income-tax Rules, as published by Protean, an authorised PAN service provider)

The form itself is in English, exactly as the issuing office prints it.

How to fill it in

  1. Fill the form in English, in block letters, one character per box. Write your PAN and Aadhaar number at the top, and stick two recent colour photographs (4.5 cm x 3.5 cm) in the spaces given, signing across the left one.
  2. Part A, items 1 to 7: name (1A) and name as per Aadhaar (1B), gender, date of birth, address (residence or office), passport number, tax number abroad and contact details. Tick the box in the left margin only against the items that you want changed, and fill those items with the correct values.
  3. Part B, items 8 to 10: father's and mother's names, and which parent's name is to be printed on the PAN card.
  4. Part C, item 11: tick the documents you attach: proof of identity, proof of address, proof of date of birth, documentary proof in support of the other changes, and a copy of the PAN.
  5. Write your name and capacity (self or representative assessee) in the declaration, then the place, date and your signature or left thumb impression in the box at the bottom right.
