Passport Annexure A: identity certificate for government and PSU employees
The form says that all central government employees, state government employees, employees of statutory bodies and public sector undertakings, and their spouse and children up to 18 years, are required to produce an identity certificate. The employer (the head of office or the certifying authority) fills it in on the office's original stationery, in duplicate, and you take it with your passport application.
Issued by: Ministry of External Affairs (Passport Seva), under the Passports Act, 1967
The form itself is in English, exactly as the issuing office prints it.
How to fill it in
- Ask your office to prepare it in duplicate on its original stationery. Strike out the options that do not apply, as the form says.
- "Certified that Shri/Smt/Miss ... Son/Wife/Daughter of Shri ...": the employee's full name and the father's or husband's name.
- "employee of (office address)": strike out temporary or permanent, then write the office address, the date of joining and the post held now.
- "Shri/Smt/Miss ... is/are a dependent family member(s) of ...": use this line when the applicant is the employee's spouse or child. Write the dependant's name and the employee's name. If the employee is applying, strike this line out.
- "Central/State Government/Public Sector undertaking/Statutory body": the certifying authority strikes out what does not apply. Then write the employee's identity card number.
- Write the office's reference number and the date. The certifying authority then writes his or her name, designation, address and telephone number and signs. The applicant's photograph on the form is to be attested by the certifying authority.
