Passport Annexure C: declaration for a minor's passport when one parent has not consented
Annexure C is the specimen declaration, on plain paper, by the parent who is applying for a passport for a minor child when the other parent's consent has not been obtained. The form lists the reasons that can be given, for example a parent living or travelling abroad, divorce or separation with custody, a pending court case, or a parent who has deserted the child. For each reason it lists the supporting papers to attach. It is submitted with the child's passport application.
Issued by: Ministry of External Affairs (Passport Seva), under the Passports Act, 1967
The form itself is in English, exactly as the issuing office prints it.
How to fill it in
- Use plain paper, as the form says. Write "I, father/mother" with your own name and strike out the one that does not apply.
- Child's details: name of the minor child and date of birth. Then the name, citizenship, passport number, contact number, email and address of the father, and the same for the mother.
- Paragraph 1: strike out mother or father as needed, and write the ARN (application reference number) of the child's passport application.
- Paragraph 2: write the name of the parent whose consent is missing, then read reasons (a) to (i). Leave the one reason that is true and put a cross mark on all the others, as the form says. Attach the supporting documents listed under that reason.
- Paragraph 3: write the child's name where the form asks. This paragraph states that the child is a citizen of India and is in your exclusive physical custody.
- Paragraphs 4 and 5: write the child's name where asked, read the undertakings (you alone would defend any court case that follows, and false information can lead to action by the Passport Authority), then sign and date.
