Passport Annexure H: prior intimation letter to your employer
Annexure H is the prior intimation (PI) letter, on plain paper, from a government, PSU or statutory body employee to the administrative office, telling it that the employee is applying for an ordinary passport. The form notes that the Passport Authority accepts it if it carries the signature and seal of the employer acknowledging receipt. Submit it to your office first, then take the acknowledged copy with your passport application.
Issued by: Ministry of External Affairs (Passport Seva), under the Passports Act, 1967
The form itself is in English, exactly as the issuing office prints it.
How to fill it in
- Top right: write the place and date.
- Address block: address the letter to your Controlling or Administrative Authority with the full postal address, PIN, telephone, fax and email.
- In the body, write the name of the Regional Passport Office where you are applying for an ordinary passport. The subject line is already printed.
- Fill in your signature, name, date of birth, designation, the office where you work, your organisation, the address of your present office and your residential address.
- The employer then signs and puts the office seal on the left, acknowledging receipt. Keep that acknowledged copy for the passport office.
