Affidavit for transfer in the case of intestate death (CHB)
Legal heirs asking CHB to transfer a dwelling unit after the allottee died without a will.

How to fill it in
- Download the form and print it. Fill it in by hand, in clear block letters, with a blue or black pen.
- Fill in the blanks the form prints: Your name, Your father's or husband's name, Your address, Name of the deceased allottee, Deceased's father's or husband's name, Date of death and the others.
- Attach with it: Print on judicial stamp paper of ₹5 or more (choose "stamp paper" below). Original death certificate of the allottee / transferee. Copy of the allotment / transfer letter or conveyance deed. ID proof, Aadhaar and PAN copies of the applicants.
- Where it goes: Chandigarh Housing Board, with the transfer application. Attest it before a Notary Public or Executive Magistrate first.
- Sign where the form asks you to, check that no blank is left empty, and keep a photocopy for yourself.
