Affidavit of applicant: duplicate allotment letter (CHB)
The applicant for a duplicate of a lost CHB letter, slip or certificate: it goes with the application.

How to fill it in
- Download the form and print it. Fill it in by hand, in clear block letters, with a blue or black pen.
- Fill in the blanks the form prints: Name of the applicant, Father's / husband's name, House number where you live now, Sector where you live now, House number allotted by CHB, Sector of the allotted house and the others.
- Attach with it: All affidavits are attested by a Notary Public / Executive Magistrate, on judicial stamp paper of ₹5 or more. Copy of the DDR (Daily Diary Report) registered with the police about the loss.
- Where it goes: Attach it to the application at the Reception Counter, Chandigarh Housing Board. Attest it before a Notary Public or Executive Magistrate; it goes on judicial stamp paper of ₹5 or more.
- Official time limit: 30 working days, not counting the public notice (CHB Citizens' Charter, section 3.3).
- Sign where the form asks you to, check that no blank is left empty, and keep a photocopy for yourself.
