How to fill it in
- Download the form and print it. Fill it in by hand, in clear block letters, with a blue or black pen.
- Fill in the blanks the form prints: Your name, Your father's or husband's name, Your address, House number, Sector, Date of signing.
- Attach with it: Self-attested copies of ID proof, Aadhaar card and PAN card of the applicant(s).
- Where it goes: Attached to the application at the CHB Reception Counter. Print on judicial stamp paper of Rs.5 or more and attest before a Notary Public or Executive Magistrate.
- Sign where the form asks you to, check that no blank is left empty, and keep a photocopy for yourself.
