Form A: proforma for Paying Guest registration (CHB)
Owner of a CHB independent house who wants to run Paying Guest accommodation.

How to fill it in
- Download the form and print it. Fill it in by hand, in clear block letters, with a blue or black pen.
- Fill in the blanks the form prints: Name of the owner / lessee, Father's or husband's name, Category / covered area of the house, Address of the premises, Contact number, Total accommodation and capacity for Paying Guests and the others.
- Attach with it: Form-A, Form-B, affidavit and indemnity bond, as per the specimens.
- Where it goes: Attached to the application at the CHB Reception Counter.
- Sign where the form asks you to, check that no blank is left empty, and keep a photocopy for yourself.
