Form B: proforma for the paying guest (CHB)
The paying guest staying in a registered CHB independent house (one form per guest).

The form itself is in English, exactly as the issuing office prints it.
How to fill it in
- Download the form and print it. Fill it in by hand, in clear block letters, with a blue or black pen.
- Fill in the blanks the form prints: Name of the paying guest, Date of birth and sex, Father's / husband's / guardian's name, with address, Permanent home address with contact number, E-mail address, Mobile number and the others.
- Attach with it: Form-A, Form-B, affidavit and indemnity bond, as per the specimens.
- Where it goes: Attached to the application at the CHB Reception Counter.
- Sign where the form asks you to, check that no blank is left empty, and keep a photocopy for yourself.
