Affidavit of beneficiaries of a registered will (CHB)
The beneficiary of a registered will asking CHB to transfer the dwelling unit of the deceased allottee.

How to fill it in
- Download the form and print it. Fill it in by hand, in clear block letters, with a blue or black pen.
- Fill in the blanks the form prints: Your name, Your father's or husband's name, Your address, Name of the deceased allottee, Dwelling unit number, Sector and the others.
- Attach with it: Print on judicial stamp paper of ₹5 or more (choose "stamp paper" below). Three self-attested photographs of the applicant(s) along with specimen signatures. Self-attested copy of ID proof and Aadhaar / PAN card of the applicant. Self-attested copy of the allotment letter / transfer letter / deed of conveyance.
- Where it goes: Attached to the application at the CHB Reception Counter. Attest it before a Notary Public or Executive Magistrate first.
- Official time limit: 25 working days, excluding the public notice (Citizens' Charter s2.3, Annex A).
- Sign where the form asks you to, check that no blank is left empty, and keep a photocopy for yourself.
