Affidavit (death case) for allotment of a small flat to the children (CHB)
Children of a deceased small-flat applicant and co-applicant, applying before the flat is allotted.

How to fill it in
- Download the form and print it. Fill it in by hand, in clear block letters, with a blue or black pen.
- Fill in the blanks the form prints: Name of the deponent, Original applicant, Original co-applicant, Jhuggi number, Block, Colony and the others.
- Attach with it: All affidavits shall be attested by a Notary Public / Executive Magistrate. Affidavits shall be on judicial stamp paper of Rs.5 or more. Original death certificate of the deceased applicant and co-applicant, and of any legal heir who has died.
- Where it goes: Attach it to the allotment application (Reception Counter, Chandigarh Housing Board). It must be on judicial stamp paper of Rs.5 or more and attested by a Notary Public or Executive Magistrate.
- Sign where the form asks you to, check that no blank is left empty, and keep a photocopy for yourself.
