How to fill it in
- Download the form and print it. Fill it in by hand, in clear block letters, with a blue or black pen.
- Fill in the blanks the form prints: First deponent, Second deponent, Prefab shed / jhuggi number, Colony / sector, Date of signing.
- Attach with it: All affidavits shall be attested by a Notary Public / Executive Magistrate. Affidavits shall be on judicial stamp paper of Rs.5 or more.
- Where it goes: Attach it to the allotment application (Reception Counter, Chandigarh Housing Board). It must be on judicial stamp paper of Rs.5 or more and attested by a Notary Public or Executive Magistrate.
- Sign where the form asks you to, check that no blank is left empty, and keep a photocopy for yourself.
