How to fill it in
- Download the form and print it. Fill it in by hand, in clear block letters, with a blue or black pen.
- Fill in the blanks the form prints: Applicant / child 1, Applicant / child 2, Applicant / child 3, Applicant / child 4, Original applicant, Original co-applicant and the others.
- Attach with it: Affidavit (death case), standard affidavit of the scheme, and indemnity bond with two witnesses and their self-attested ID proofs. Self-attested photocopies of ID proof, Aadhaar cards and PAN card of the applicants / children, and of their voter cards. Self-attested copy of ID proof of the witnesses of the indemnity bond. Receipts of publication and processing charges (online at the CHB website or at any HDFC Bank branch). Original death certificate of the deceased applicant and co-applicant, and of any legal heir who has died.
- Where it goes: Reception Counter, Chandigarh Housing Board (addressed to the Secretary, CHB).
- Sign where the form asks you to, check that no blank is left empty, and keep a photocopy for yourself.
