Application for transfer of licence of a small flat to the children (CHB)
Children of a deceased small-flat allottee and co-allottee, where the flat is already allotted.

How to fill it in
- Download the form and print it. Fill it in by hand, in clear block letters, with a blue or black pen.
- Fill in the blanks the form prints: Applicant / child 1, Applicant / child 2, Applicant / child 3, Applicant / child 4, Original allottee, Original co-allottee and the others.
- Attach with it: Affidavit (death case), and indemnity bond along with two witnesses and their self-attested ID proofs. Three self-attested photographs of the applicant(s) along with specimen signatures. Self-attested photocopies of ID proof, Aadhaar cards and PAN card of the applicants / children. Self-attested copy of ID proof of the witnesses of the indemnity bond. Receipts of publication and processing charges (online at the CHB website or at any HDFC Bank branch). Original death certificate of the allottee and co-allottee, and of any legal heir who has died.
- Where it goes: Reception Counter, Chandigarh Housing Board (addressed to the Secretary, CHB).
- Official time limit: 25 working days, excluding the public notice (Citizens Charter, section 2.4).
- Sign where the form asks you to, check that no blank is left empty, and keep a photocopy for yourself.
