Affidavit for transfer on an un-registered Will (CHB)
The beneficiary of an un-registered Will of a deceased CHB allottee / transferee.

How to fill it in
- Download the form and print it. Fill it in by hand, in clear block letters, with a blue or black pen.
- Fill in the blanks the form prints: Your name, Your father's or husband's name, Your address, Name of the deceased allottee, Deceased's father's or husband's name, Dwelling unit number and the others.
- Attach with it: Print on judicial stamp paper of ₹5 or more; attest before a Notary Public or Executive Magistrate.
- Where it goes: Attached to the transfer application at the CHB Reception Counter. Print it on judicial stamp paper of ₹5 or more and have it attested by a Notary Public or Executive Magistrate.
- Official time limit: 25 working days, excluding the public-notice period (CHB Citizens' Charter, section 2.3 and Annex A).
- Sign where the form asks you to, check that no blank is left empty, and keep a photocopy for yourself.
