1.3 The signature I now use (the "New Signature") appears on: My passport, my Aadhaar-linked documents and my recent cheques.
1.4 The two signatures differ because: My handwriting has changed over the years, and I now sign with my full name instead of initials.

2.1 Both the Old Signature and the New Signature are mine, and I ask that the New Signature be recorded in place of the Old Signature.

5.1 Every statement made above is true to my own knowledge and belief, and I have concealed nothing material. I am aware that making a false declaration in an affidavit is an offence.
