1.2 Correction of the name is required in my elector record.
1.3 The entry as it now stands is: Gurpreet Sing
1.4 The entry as it should read is: Gurpreet Singh
1.5 The reason for the correction is: My name was spelt wrongly when the roll was prepared.

2.1 In support of the correct particulars, I rely on and am submitting the following document: Aadhaar card No. XXXX XXXX 1234, issued by UIDAI
- 3. HOW THIS DECLARATION IS BEING FILED
3.1 I am submitting this declaration, together with the supporting document, through the Election Commission of India's online voter services, uploading a scanned copy of the supporting document.

4.1 Every statement made above is true to my own knowledge and belief. I have not sought this correction to assume a false identity or to defeat any law, and I have concealed no material fact.
4.2 I am aware that the Electoral Registration Officer may verify this declaration and the supporting document, may make an enquiry at my address, and may ask for a further document, and that furnishing false particulars for the electoral roll is an offence.
