PARENTAL CONSENT FOR MINOR TO TRAVEL ABROAD

I/We, Rohit Malhotra (Father) and Neha Malhotra (Mother), of House No. 302, Sector 9, Panchkula, being Both parents named in this document, jointly, The minor’s father is deceased; the mother is the sole surviving parent and natural guardian, do hereby solemnly affirm, declare and give consent as under:

  1. 1. That Aryan Malhotra, born on 1 April 2026, holding passport number N6789012 ("the Minor"), is our/my child.
  1. 2. That we/I hereby give full and free consent for the Minor to travel to Singapore from 2 April 2026 to 3 April 2026, for the purpose of Family holiday.
  1. 3. That the Minor is travelling alone, as an unaccompanied minor under the airline’s own programme, namely Mr. Sandeep Kapoor, maternal uncle, passport number N7890123.
  1. 4. That we/I authorise the airline, immigration authorities, and any other authority concerned to allow the Minor to travel and to rely on this consent for that purpose.
  1. 5. That we/I remain the lawful parent(s)/guardian of the Minor and retain full parental responsibility, and that this consent is limited to the travel described above.
  1. 6. That the contents of this consent are true and correct to our/my knowledge and belief.

VERIFICATION

Verified at Panchkula on 3 April 2026 that the above is true and correct to our/my knowledge, that no part of it is false and that nothing material has been concealed therefrom.
