How to fill it in
- Download the form and print it. Fill it in by hand, in clear block letters, with a blue or black pen.
- Fill in the blanks the form prints: Site / plot number, Sector, Area of the site, File number, Name of the lessee / applicant, Father's or husband's name and the others.
- Attach with it: Affidavit of the applicant(s) as per specimen, on judicial stamp paper of ₹5 or more, attested by a Notary Public or Executive Magistrate. Three self-attested photographs of the applicant(s) along with specimen signature. Self-attested copy of ID proof, Aadhaar card and PAN card of the applicant. Demand draft of the conversion fee (to be submitted on demand, after the case is processed). Self-attested copy of allotment letter and physical possession slip. Self-attested copy of transfer letter, if any. Self-attested copy of No Due Certificate. Details of payments after the No Due Certificate was issued. No Objection Certificate from the mortgagee, if the property is mortgaged. Receipt of processing charges (deposited online at the CHB website or at any HDFC Bank branch).
- Where it goes: Reception Counter, Chandigarh Housing Board (addressed to the Secretary, CHB).
- Official time limit: 30 working days, excluding public notice (CHB Citizens’ Charter, section 3.1).
- Sign where the form asks you to, check that no blank is left empty, and keep a photocopy for yourself.
