Affidavit (death case) for transfer of licence of a small flat (CHB)
Children of a deceased small-flat allottee and co-allottee, where the flat is already allotted.

How to fill it in
- Download the form and print it. Fill it in by hand, in clear block letters, with a blue or black pen.
- Fill in the blanks the form prints: Name of the deponent, Original allottee, Original co-allottee, Small flat number, Sector, Child 1 and the others.
- Attach with it: All affidavits shall be attested by a Notary Public / Executive Magistrate. Affidavits shall be on judicial stamp paper of Rs.5 or more. Original death certificate of the allottee and co-allottee, and of any legal heir who has died.
- Where it goes: Attach it to the licence-transfer application (Reception Counter, Chandigarh Housing Board). It must be on judicial stamp paper of Rs.5 or more and attested by a Notary Public or Executive Magistrate.
- Official time limit: 25 working days, excluding the public notice (Citizens Charter, section 2.4).
- Sign where the form asks you to, check that no blank is left empty, and keep a photocopy for yourself.
