e-Stamp application form, Punjab (SHCIL Form 4)
In Punjab, stamp paper is now an e-Stamp certificate. You fill in this form, pay the stamp duty at an authorised e-Stamp centre (a bank, a SHCIL office or an authorised collection centre), and the centre prints the e-Stamp certificate for you.

How to fill it in
- Top of the form: write the date. "ACC Name and ID" is the e-Stamp centre's own name and code, so ask at the counter if you do not know it.
- "Document Description": the type of document the stamp paper is for, for example "Agreement", "Affidavit" or "Rent Agreement".
- "Stamp Duty", "Social Infrastructure Cess" and "Total Stamp Duty": the amounts in rupees, rounded to the nearest rupee. Our free stamp paper value finder and stamp duty calculator can help you work these out.
- "Property Description / Property Address": up to 100 characters. Write the consideration amount (the price or value in the document) if there is one.
- "Area of Property": fill in ONE unit only (acre, kanal, marla, bigha, square feet, square yard or square metre), or tick "Not Applicable" if there is no property, for example for an affidavit.
- First party, second party and purchaser: each name and address in up to 50 characters, with a phone number. Then tick who is paying the stamp duty.
- "Mode of Payment": tick cash, cheque, DD, pay order, NEFT, RTGS or account transfer. For a cheque, DD or transfer, fill in the bank, branch and number. For cash, fill in the notes table. Write the amount in words.
- Declaration: tick (i) for a new e-Stamp certificate, or (ii) for an additional certificate that continues an earlier one (then write its number and date). Sign and date it. Leave the "For Office use only" boxes empty.
- Before you sign the preview the centre shows you, check every detail. Once the e-Stamp is generated nothing can be changed, and the centre cannot cancel or refund it. A refund for a spoiled or unused e-Stamp is claimed from the District Collector under the Punjab e-Stamp Rules, 2014. Keep the receipt at the bottom of the form.
