When you need it

You (or your parent) draw a pension from an Indian source — government, EPFO, a bank pension, or a similar scheme — and now live abroad, where the pension-disbursing authority's own biometric or Jeevan Pramaan system may not reach. This sworn affidavit/life certificate, attested before a notary or the nearest Indian Consulate, confirms you are alive as on the date stated, so pension credit is not stopped or suspended for want of a periodic life certificate.

Confirm with your specific pension-disbursing bank, treasury or EPFO office which exact certificate format they require and how often — some accept a notarised affidavit in this form, others insist on their own printed proforma or a Digital Life Certificate through an authorised centre.

Notarisation

Sworn/attested before a Notary Public where you live, or before the nearest Indian Consulate/Embassy, with the pensioner appearing in person and showing photo identification — some authorities specifically want consular attestation rather than a local notary for a pensioner abroad, so confirm before the appointment.

Witnesses

Not required by law; some authorities separately want a person who can identify you before the attesting officer.

LIFE CERTIFICATE / EXISTENCE AFFIDAVIT

I, Surjit Singh Sandhu, born on 1 April 2026, presently residing at 9 Rosewood Court, Auckland 1010, New Zealand, holding passport number N7654321, do hereby solemnly affirm, declare and certify as under:

1. That I am a pensioner under Pension Payment Order / member reference number PB/0456789, my pension being credited to Account No. XXXXXXXX2345, State Bank of India, Amritsar.

2. That this certificate is furnished for the information of the pension-disbursing bank branch named above, namely Railway Pension Disbursing Officer, Ferozepur.

3. That I, the above-named pensioner, am alive and in existence as on 2 April 2026, and that I personally appeared before the attesting officer named below on that date for the purpose of this certificate.

4. That I undertake to inform the disbursing authority named above promptly of any change in my address or bank account particulars.

5. That the contents of this certificate are true and correct to my knowledge and belief.
