A sworn affidavit of loss, combined with an indemnity, for a passport or a company/body corporate share certificate that has been lost, stolen or destroyed — used to support either (a) a reissue or duplicate passport application under the Passport Rules, 1980, at a Passport Seva Kendra or Regional Passport Office, or (b) an application to a company, or its Registrar and Transfer Agent, for a duplicate share certificate under section 46 of the Companies Act, 2013 read with Rule 6 of the Companies (Share Capital and Debentures) Rules, 2014. Records the loss, the search made, whether a police report (FIR) or an online lost-property complaint was lodged, and whether a public notice was published, and indemnifies the receiving authority or company against the consequences of issuing a duplicate. Not for a lost marksheet, degree or academic certificate — use the Duplicate Marksheet / Lost Certificate Affidavit for that. Not for a lost title deed or other property document — use the Affidavit and Indemnity Bond for a Lost Title Deed for that. Not for a lost housing-society membership share certificate — use the Duplicate Share Certificate Application, which follows that society's own bye-laws, for that. Not for dematerialised (demat) shares, which have no physical certificate to lose — a demat holding is restored through your Depository Participant, not through this affidavit.
