For an applicant seeking a Backward Class (BC) or Scheduled Caste (SC) certificate from the Tehsildar/SDM in Punjab or the Deputy Commissioner's office in Chandigarh, where the application asks for a sworn affidavit supporting the claimed caste, native place and — for a Backward Class claim only — that the family is outside the creamy layer. A Scheduled Caste claim carries no creamy layer exclusion: the creamy layer test applies only to Backward Classes/OBCs, following Indra Sawhney v. Union of India. This affidavit branches its declarations by which of the two is being claimed, so the creamy-layer questions appear only for a Backward Class claim. Used for admissions, recruitment and promotion where caste-based reservation applies, and renewed with a fresh affidavit whenever the certificate itself is renewed — a Backward Class certificate is commonly revalidated periodically; a Scheduled Caste certificate, once issued, is ordinarily not time-limited in the same way, but confirm the current requirement with the issuing office.

Registration

Not a registrable instrument. Annexed to the caste certificate application filed with the Tehsildar/SDM (Punjab) or the Deputy Commissioner's office (Chandigarh), which independently verifies the claim — commonly through a report from the Patwari or Lambardar of the applicant's native village, or the Circle Revenue Officer — before issuing the certificate. There is no Sub-Registrar step.

Notarisation

Sworn before a Notary Public (Notaries Act, 1952) or an Executive Magistrate/Oath Commissioner. Where sworn on the spot at a Suwidha Kendra or Sampark counter, that counter's own attesting officer may accept the affidavit in their presence in place of prior notarisation — confirm with the specific counter first.
