Take the affidavit, with your identity proof, to a notary or an Oath Commissioner, and sign in front of them. Not for: an income or residence affidavit for use outside Haryana (use the Income Affidavit or the Residence Proof Affidavit); a family ID correction made by an official on the portal without an affidavit.
