You need to establish that you are a bonafide Himachali — for a government post or promotion reserved for domiciles of the State, admission under the Himachal Pradesh domicile quota, or another purpose that calls for proof of bonafide Himachali status. This is the application, made on the departmental declaration commonly called Form-F, together with the sworn affidavit that supports it, filed with the Sub Divisional Officer (Civil) or the Tehsildar of the area where you or your family ordinarily reside. The certificate itself, once the Patwari's verification is complete, issues in Form-G. Not for proving you are an "agriculturist" under section 118 of the Himachal Pradesh Tenancy and Land Reforms Act, 1972 — that is a different and narrower test, turning on who actually cultivates land, not on how long a family has lived in the State. A bonafide Himachali who does not personally cultivate land may still need section 118 permission to buy agricultural land; see the note in the fields below before assuming this certificate changes that.

The application is filed with the Sub Divisional Officer (Civil) or the Tehsildar having jurisdiction over the Applicant's place of ordinary residence, or lodged through the Himachal Pradesh e-District portal where that route is open for this service — confirm the current portal address and whether online filing is accepted in the district concerned before relying on it.

Notarisation

The supporting affidavit is sworn before a Notary Public, an Oath Commissioner, or an Executive Magistrate. The Patwari's verification report on residence, which the revenue officer calls for separately after the application is filed, is a departmental step and not a notarisation — it cannot be supplied by the Applicant and should not be confused with the affidavit.
