When you need it

Use this when you want to move your health insurance policy from your present insurer to another one at renewal, and keep the credit for the cover you have already earned, such as waiting periods you have served and a continuous record of cover. You write to the new insurer, with a copy to the present one.

The letter gives the present policy and how long it has run, the persons covered, the claim history, the cover you want, the existing illnesses of the persons covered, and your authority for the new insurer to ask the present insurer for the records. Portability must be asked for within a stated time before your present policy is due for renewal, and the rules also say how soon the insurers must respond; check both with the new insurer well in advance and do not let the present policy lapse before the new one is confirmed. The new insurer decides your application on its own terms and may decline it or add conditions; the credit it gives for waiting periods and sum insured is what the portability rules allow, not more. Compare the premium, the waiting periods, the room-rent and other limits, and the network hospitals before you move. The new insurer will have its own portability form; fill it in and enclose it with this letter. Not a claim — use the Health Insurance Claim Letter. Not a request to end a policy for a refund — use the Policy Surrender or Free-Look Request. Not for changing the nominee — use the Nominee Change Declaration.

What follows is written for Punjab and Chandigarh. It states the position there rather than describing how it varies, so the amounts below are the ones that apply — worth confirming at the sub-registrar’s office, because rates change. If the property is anywhere else in India, none of this is yours.

Stamp duty

Punjab and Chandigarh: no stamp duty is payable. A letter asking an insurer to perform its own contract creates, transfers or ends no right, so it is not a chargeable instrument under the Indian Stamp Act, 1899. Plain paper is enough.

Registration

Punjab and Chandigarh: not registrable. It touches no immovable property, so section 17 of the Registration Act, 1908 does not reach it. Keep proof of the day it was lodged: the stamped copy, the email delivery receipt or the post receipt.

Notarisation

Punjab and Chandigarh: no notarisation is needed. If the insurer asks for a sworn statement, it will say which of its papers it wants sworn. The new insurer may ask the proposer to sign its own proposal and portability forms.

Witnesses

Punjab and Chandigarh: no witness is needed. The signatory signs alone.

Sir / Madam,

1. MY PRESENT POLICY

1.1 I hold an individual health policy with ABC Health Insurance Company Limited, number P/141234/01/2026/003318, with a sum insured of ₹5,00,000 (Rupees Five Lakh only). My health cover has run without a break since 2 April 2026, and the present term ends on 3 April 2026.

1.2 The persons covered are: Harjinder Singh Gill (self), 14 March 1978. Manpreet Kaur Gill (wife), 2 August 1981. Aman Singh Gill (son), 9 June 2010.

1.3 No claim has been made under the present policy.

2. MY REQUEST

2.1 I ask you to issue me a health policy under your plan Family Health Optima, with a sum insured of ₹10,00,000 (Rupees Ten Lakh only), to begin on the day my present term ends.

2.2 I ask you to give me credit under the new policy for the cover already earned under my present policy, including the waiting periods already served, to the extent the portability rules allow.

2.3 I authorise you to ask ABC Health Insurance Company Limited for the details of my present policy, its claim history and the medical history of the persons covered, and I authorise ABC Health Insurance Company Limited to give them to you.

2.4 I understand that you will decide on my request on your own terms, and that you may accept it, decline it or add conditions.

3. EXISTING ILLNESSES

3.1 The existing illnesses or treatments of the persons covered are: Harjinder Singh Gill: type 2 diabetes since 2019, on regular medication. No others.

3.2 I have given you, or will give you on your own form, every fact you ask about, and I have kept back none that you would reasonably want to know.

4. TIMING

4.1 My present term ends on 3 April 2026. Please decide before then and tell me in writing, so that my cover does not lapse. I am sending a copy of this letter to ABC Health Insurance Company Limited.

4.2 I will not let my present policy lapse until you confirm the new policy in writing.
