When you need it

Your vehicle's Registration Certificate, or its motor insurance policy or cover note, has been lost, stolen, destroyed or badly mutilated, and you need a sworn affidavit of loss to support an application for a duplicate — of one, or of both together, since the two are often lost at the same time. A duplicate Registration Certificate is granted under Rule 47 of the Central Motor Vehicle Rules, 1989, made under the Motor Vehicles Act, 1988, which applies uniformly across India; what is jurisdiction-specific here is the Registering Authority you file before — in Punjab the Sub-Divisional Motor Vehicle Officer or District Transport Officer of the district, in Chandigarh the Registering Authority, Union Territory of Chandigarh — and where this affidavit is notarised.

A duplicate insurance policy or cover note is governed by the Insurance Act, 1938 and IRDAI's own regulations, which are central and not jurisdiction-specific at all; it is issued by the insurer named in the affidavit, not by any Punjab or Chandigarh authority, on whatever proof of loss the insurer's own process currently asks for.   1. 3. CIRCUMSTANCES OF LOSS

3.1 The Registration Certificate was in my wallet, which was lost while travelling by bus from Jalandhar to Amritsar on 5 September 2026; despite a diligent search it has not been found. I have not found the original since, I have not misused it, and I have not pledged, hypothecated or handed it to any other person as security; if I find it after a duplicate is issued, I undertake to surrender it to SDM-cum-Registering & Licensing Authority, Jalandhar.

3.2 I lodged a report of the loss, particulars of which are: DDR No. 214/2026 dated 6 September 2026, Police Station Division No. 4, Jalandhar.

  1. 4. DECLARATION

4.1 Every statement made above is true to my own knowledge, information and belief, and I have concealed nothing material to this application. I am aware that furnishing false information in support of this application is an offence under the Motor Vehicles Act, 1988, or, as the case may be, is a false declaration to the insurer, and that the duplicate sought is liable to be refused, or cancelled, if any statement made here is found false.

VERIFICATION

Verified at Jalandhar on 1 April 2026 that the contents of paragraphs 1 to 4 above are true and correct to my own knowledge, that no part of it is false, and that nothing material has been concealed.
