When you need it

You are an employee in Punjab or Chandigarh and want to tell your employer, in your own letter, who you nominate to receive your gratuity and your provident fund if you die, along with your family particulars. You can also use it when your family changes — a marriage, a birth, a death — and the nomination has to be updated.

This letter is your covering declaration. It is not the nomination itself: the employer, and the provident fund office for the provident fund, each require a nomination in their own prescribed form or online process, and a nomination is valid only in that form. Write this letter to go with those, and ask your employer's HR for the current forms. This document does not copy any of them. Where you have a family, the rules generally require the nominees to be members of your family, and a nomination of someone outside it may be ineffective. Check this with HR or a lawyer if you want to nominate anyone else. Not a will, which is a separate document that deals with all your property. A nomination is not a will and does not by itself decide who owns the money.

What follows is written for Punjab and Chandigarh. It states the position there rather than describing how it varies, so the amounts below are the ones that apply — worth confirming at the sub-registrar’s office, because rates change. If the property is anywhere else in India, none of this is yours.

Stamp duty

Not chargeable to stamp duty under the Indian Stamp Act, 1899, in Punjab or Chandigarh — it is a letter, not an instrument that creates or transfers any right. Print it on the employer's letterhead. The employer’s or provident fund office’s own nomination form may have its own requirements for a witness; follow it.

Registration

Not registrable. Section 17 of the Registration Act, 1908 has no application to a letter of this kind.

Notarisation

Not required. What matters is proof that it reached the other side: hand delivery against a signed acknowledgement, registered post, or a dated email. Keep that proof with the file.

Witnesses

None required.

House No. 214, Sector 15-A, Chandigarh 160015

Date: 2 April 2026

To,
The Head of Human Resources
Northline Analytics Private Limited
Plot No. 8, Phase 8-B, Industrial Area, S.A.S. Nagar (Mohali) 160055

Sub: Nomination for gratuity and provident fund (Employee ID NA-0231)

Dear Sir/Madam,

I, Rajesh Bansal, son/daughter/wife of Mohan Lal Bansal, working with Northline Analytics Private Limited as Field Sales Executive since 1 April 2026, provident fund number (UAN) 100123456789, give you the following declaration.

  1. 1. MY FAMILY. Wife Sunita Bansal (38), son Aarav Bansal (9), mother Meena Bansal (66). I have no other dependants.
  1. 2. MY NOMINEES. I nominate the following persons to receive the gratuity and the provident fund and any other amount payable by the Company or the Fund on my death, in the shares shown:

(a) Sunita Bansal, wife, 38 years, share 50 percent.
(b) Aarav Bansal, son, 9 years, share 25 percent.
(c) Meena Bansal, mother, 66 years, share 25 percent.

The shares add up to one hundred percent.

NOMINEE WHO IS A MINOR. Where a nominee is a minor, the money for that nominee may be paid to Sunita Bansal, mother of Aarav, an adult who will hold it for the child.

  1. 3. PRESCRIBED FORMS. I understand that this letter is my covering declaration only. The Company's gratuity nomination and the provident fund nomination must also be made in the form or process that the Company and the Provident Fund office prescribe, and I will complete and sign those forms, and the online nomination where the Fund asks for it, as soon as they are given to me. Please send me the current forms.
  1. 4. FAMILY NOMINEES. I understand that, where I have a family, the nominees must be members of my family as the rules provide, and that a nomination that does not meet the rules may not be given effect.
  1. 5. CHANGES. I will tell the Company in writing, and make a fresh nomination in the prescribed form, if my family changes by marriage, birth, death or otherwise, or if I wish to change my nominees. I understand that a nomination does not by itself decide who owns the money, and that it does not replace a will.
  1. 6. The particulars above are true and complete to the best of my knowledge.

Yours faithfully,
