When you need it

The first letter to a life insurer after the person who held the policy has died — it tells the insurer of the death, gives the policy and death details and claims the money as nominee or, where there is none, as a legal heir. It also asks the insurer to register the claim, send you its claim forms and tell you in writing which papers it needs.

Answer the questions from the policy document and the death certificate: policy number, whether it was in force, how the death happened, and your place under the policy. Then attach the death certificate, the original policy document or a note that it is lost, your identity and address proofs with the Aadhaar number masked, and whatever else the insurer lists after you write. Where the death was by accident or in unusual circumstances, the insurer may ask for further papers; the letter lets you list them. No stamp duty or registration is needed for the letter. Send it to the insurer's branch or claims office by hand against an acknowledgement, or by registered post, and keep the proof. Not for a hospital or medical-expense claim (use the Health Insurance Claim Letter) or for a provident fund or pension claim (see the EPF and Pension Death Claim Covering Letter).

What follows is written for Punjab and Chandigarh. It states the position there rather than describing how it varies, so the amounts below are the ones that apply — worth confirming at the sub-registrar’s office, because rates change. If the property is anywhere else in India, none of this is yours.

Stamp duty

Punjab and Chandigarh: no stamp duty is payable. A letter telling an insurer of a death and claiming the policy money creates, transfers or extinguishes no right, so it is not an instrument within section 2(14) of the Indian Stamp Act, 1899, and the claimant signs it alone. The insurer may separately ask for an indemnity bond or an affidavit on stamp paper; those are other papers, and their duty is not part of this letter.

Registration

Punjab and Chandigarh: not registrable. A claim for policy money is a claim for money, not for immovable property, so section 17 of the Registration Act, 1908 does not reach this letter. Keep a copy and the insurer's acknowledgement.

Notarisation

Punjab and Chandigarh: no notarisation is needed for the letter. The insurer's claim forms may ask for a signature to be attested, and an affidavit may need to be sworn; follow what the form and the insurer say.

Witnesses

Punjab and Chandigarh: no witness is needed for the letter. The claimant signs alone; any witness or attestation on the insurer's claim form is the insurer's requirement.

LIFE INSURANCE DEATH CLAIM LETTER

Intimation of death and claim under a life insurance policy

Date: 2 April 2026, at Chandigarh

To: The Manager, Claims, Life Insurance Corporation of India, Sector 17, Chandigarh

From: Aarav Singh, Sector 35, Chandigarh. Telephone: +91 XXXXX XXXXX. Email: aarav@example.com.

Subject: Death of Harpreet Singh, policy no. 123456789 — intimation and claim

Sir or Madam,

  1. 1. The policy. Harpreet Singh held policy no. 123456789 with you. The policy was in force on the date of death. Other policies of the same person with you: Policy 987654321, a money-back plan.
  1. 2. The death. Harpreet Singh died on 1 April 2026 at Chandigarh. The death certificate is DTH/2026/118, and a copy is enclosed. The circumstances of the death: Died at home after a heart attack; treated earlier at Fortis Hospital, Mohali. The person was last treated by Fortis Hospital, Mohali.
  1. 3. My claim. My relationship to the person who died: Spouse. I claim as the nominee recorded in the policy. I claim the money payable under the policy on the death of the life assured.
  1. 4. The legal heirs. Apart from me, the other legal heirs of Harpreet Singh are: Naina Singh and Meera Singh, Chandigarh.
  1. 5. What I ask you to do.

(a) Register this letter as the intimation of the death and of my claim, and give me the claim reference number.

(b) Send me your claim forms, and tell me in writing every paper you need from me, including any you need because of the way the death happened.

(c) Tell me the time within which you will decide the claim and the rule on which you rely.

(d) Tell me what is payable under the policy and how it is worked out, including any bonus, and any deduction you make.

(e) Pay the amount due to the account described in paragraph 6.

(f) If you reject the claim or any part of it, give me the reasons in writing and tell me the remedy open to me.

  1. 6. Where to pay. Please pay the amount to the account held by Aarav Singh with State Bank of India, Sector 35, Chandigarh, account ending 4821. I will give the full account number on your own form and not in this letter.
  1. 7. What I accept. I understand that a nominee is paid so that you are safely discharged, and that this does not by itself decide who owns the money as against the other legal heirs. If you pay me and it is later found that someone else had a better right, I will account to that person for what I received.
  1. 8. If the claim is not settled. If you reject the claim or do not reply within the time allowed, I will take the matter to your grievance redressal officer and then to the Insurance Ombudsman, and I am free to bring a consumer complaint before the District Consumer Disputes Redressal Commission.

Please acknowledge this letter and tell me the name and telephone number of the officer who will handle the claim.
