Sworn before a Notary Public (Notaries Act, 1952) or an Executive Magistrate/Oath Commissioner. Where sworn on the spot at a Sampark centre (Chandigarh), that counter's own attesting officer may accept the affidavit in their presence in place of prior notarisation — confirm with the specific counter first.

CHANDIGARH RESIDENCE CERTIFICATE AFFIDAVIT

I, Simranjit Kaur, daughter of Sh. Gurmeet Singh, resident of House No. 212, Guru Nanak Nagar, Jalandhar, Punjab 144001, holding Aadhaar 4444 5555 6666, ordinarily resident within the Union Territory of Chandigarh (the "Deponent" or "I"), do hereby solemnly affirm and declare as follows:

1. THE PERSON AND THE RESIDENCE

1.1 The Residence Certificate is sought for: myself.

1.2 That person has been residing in the Union Territory of Chandigarh, at House No. 1421, Sector 38-B, Chandigarh 160036, since 1 April 2026.

1.3 That person has been residing there in a house or flat owned by me or my family.

The periods, if any, during which that person was away from Chandigarh since that date are: Two months in 2022 with relatives in Ludhiana; otherwise no absence.

Before moving to that address, that person lived at: Village Raipur Kalan, Tehsil Kharar, S.A.S. Nagar (Mohali), Punjab.

2. PROOF OF RESIDENCE

2.1 In support of the above, I annex or will produce the following papers: Electricity bill, ration card, Aadhaar, school records showing the Chandigarh address.

3. PURPOSE

3.1 This affidavit is sworn in support of an application for a Residence Certificate in Chandigarh, needed for: Admission to Class 11, Government Model Senior Secondary School, Sector 35.

3.2 I undertake to inform the issuing authority if the address or the residence stated above changes before the certificate is issued, and I understand that the certificate may be withheld or cancelled if any statement made here is found false.

4. DECLARATION

4.1 Every statement made above is true to my own knowledge and belief, and I have concealed nothing material to this application. I am aware that making a false declaration in an affidavit is an offence.

Verified at Chandigarh on 2 April 2026 that the contents of paragraphs 1 to 4 above are true and correct to my own knowledge, that no part of it is false, and that nothing material has been concealed.
