A sworn statement of your current residential address, for a purpose that asks for one and where a utility bill, rent agreement or ration card is unavailable or is not by itself accepted — bank KYC, school or college admission, a gas or electricity connection, an address correction, or as one of the papers a passport or employment police verification asks for. Not for a Domicile Certificate application, which needs proof of a qualifying period of continuous residence in the State or Union Territory rather than a statement of your present address — use the Domicile Certificate Supporting Affidavit for that.

Sworn before a Notary Public (Notaries Act, 1952) or an Executive Magistrate/Oath Commissioner. Some recipients — a school admission counter, for instance — accept a plain signed declaration without notarisation; confirm which the particular recipient wants before the appointment.

A few recipients ask for the affidavit to be countersigned by a municipal councillor, a Lambardar, or a resident welfare association office-bearer who can vouch for the address — confirm with the specific recipient whether that is wanted in addition to the notarial attestation.
