A sworn statement of your current residential address, for a purpose that asks for one and where a utility bill, rent agreement or ration card is unavailable or is not by itself accepted — bank KYC, school or college admission, a gas or electricity connection, an address correction, or as one of the papers a passport or employment police verification asks for. Not for a Domicile Certificate application, which needs proof of a qualifying period of continuous residence in the State or Union Territory rather than a statement of your present address — use the Domicile Certificate Supporting Affidavit for that.
What follows is written for Punjab and Chandigarh. It states the position there rather than describing how it varies, so the amounts below are the ones that apply — worth confirming at the sub-registrar’s office, because rates change. If the property is anywhere else in India, none of this is yours.
Stamp duty
Punjab: Article 4, Schedule 1-A to the Indian Stamp Act, 1899 as applicable to Punjab, non-judicial stamp paper, commonly Rs 10, e-stamped via SHCIL (shcilestamp.com) or bought from a licensed vendor. Chandigarh: Article 4, Schedule I to the Indian Stamp Act, 1899 as extended to the Union Territory, the same nominal denomination, from SHCIL or the Treasury, Sector 17. Several recipients also accept a plain self-declaration without a notary where their own form says so — check the specific form before buying stamp paper.
Registration
Not a registrable instrument. Filed with, or annexed to the application of, whichever bank, school, department or office has asked for it. There is no Sub-Registrar step.
Notarisation
Sworn before a Notary Public (Notaries Act, 1952) or an Executive Magistrate/Oath Commissioner. Some recipients — a school admission counter, for instance — accept a plain signed declaration without notarisation; confirm which the particular recipient wants before the appointment.
Witnesses
None required by law. A few recipients ask for the affidavit to be countersigned by a municipal councillor, a Lambardar, or a resident welfare association office-bearer who can vouch for the address — confirm with the specific recipient whether that is wanted in addition to the notarial attestation.
