A sworn declaration of a date of birth, for use where a birth certificate was never issued, cannot be traced, or the receiving office asks for a supporting affidavit alongside whatever other proof of age exists — school or college admission, a first employment record, a pension or provident fund claim, or an application where the birth was never registered with the municipal or panchayat authority at the time. Sworn by the person themself for their own date of birth, or by a parent or guardian for a minor's. Not for correcting a date of birth already recorded in your Aadhaar — use the Aadhaar Name/DOB Correction Declaration, which is UIDAI's own correction process, for that.
What follows is written for Punjab and Chandigarh. It states the position there rather than describing how it varies, so the amounts below are the ones that apply — worth confirming at the sub-registrar’s office, because rates change. If the property is anywhere else in India, none of this is yours.
Stamp duty
Punjab: Article 4, Schedule 1-A to the Indian Stamp Act, 1899 as applicable to Punjab, non-judicial stamp paper, commonly Rs 10, e-stamped via SHCIL (shcilestamp.com) or bought from a licensed vendor. Chandigarh: Article 4, Schedule I to the Indian Stamp Act, 1899 as extended to the Union Territory, the same nominal denomination, from SHCIL or the Treasury, Sector 17.
Registration
Not a registrable instrument. Filed with, or annexed to the application of, whichever school, employer, department or office has asked for it. There is no Sub-Registrar step.
Notarisation
Sworn before a Notary Public (Notaries Act, 1952) or an Executive Magistrate/Oath Commissioner. A school admission office will sometimes accept a plain signed declaration instead; confirm which the particular recipient wants before the appointment.
Witnesses
None required by law; the affidavit is authenticated by the notary's or attesting officer's seal alone. A school or an employer occasionally asks for a supporting letter from a doctor or a village elder in addition — that is a separate paper, not a witness to this one.
