When you need it

A sworn declaration of a date of birth, for use where a birth certificate was never issued, cannot be traced, or the receiving office asks for a supporting affidavit alongside whatever other proof of age exists — school or college admission, a first employment record, a pension or provident fund claim, or an application where the birth was never registered with the municipal or panchayat authority at the time. Sworn by the person themself for their own date of birth, or by a parent or guardian for a minor's. Not for correcting a date of birth already recorded in your Aadhaar — use the Aadhaar Name/DOB Correction Declaration, which is UIDAI's own correction process, for that.
