When you need it

For an ex-serviceman, a member of the Ex-servicemen Contributory Health Scheme (ECHS), or a serving or retired Government employee (or a member of their family) who must show which family members depend on them — where an office, a medical scheme or a department asks for a sworn affidavit naming the dependants and the way they depend. Filed with the Sewa Kendra, edistrict.punjabgovt.gov.in or the Tehsildar/SDM in Punjab, or the Deputy Commissioner's office or a Sampark centre in Chandigarh, or direct with the scheme or department that asked for it.

What follows is written for Punjab and Chandigarh. It states the position there rather than describing how it varies, so the amounts below are the ones that apply — worth confirming at the sub-registrar’s office, because rates change. If the property is anywhere else in India, none of this is yours.

Stamp duty

Punjab: Article 4, Schedule 1-A to the Indian Stamp Act, 1899 as applicable to Punjab, non-judicial stamp paper, commonly Rs 10, e-stamped via SHCIL (shcilestamp.com) or bought from a licensed vendor. Chandigarh: Article 4, Schedule I to the Indian Stamp Act, 1899 as extended to the Union Territory, non-judicial stamp paper, commonly Rs 10, from SHCIL or the Treasury, Sector 17.

Registration

Not a registrable instrument. Annexed to, or filed together with, the application to the office, medical scheme or department that asked for it — an Ex-Servicemen records office, an ECHS polyclinic or the Station Headquarters, or the employee's own department — which checks the list against its records. Where a Tehsildar/SDM (Punjab) or the Deputy Commissioner's office (Chandigarh) issues the certificate, they may also call for a Patwari or field report. There is no Sub-Registrar step.

Notarisation

Sworn before a Notary Public (Notaries Act, 1952) or an Executive Magistrate/Oath Commissioner. Where sworn on the spot at a Sewa Kendra (Punjab) or Sampark centre (Chandigarh), that counter's own attesting officer may accept the affidavit in their presence in place of prior notarisation — confirm with the specific counter first.

Witnesses

None required by law for the affidavit itself; it is authenticated by the notary's or attesting officer's seal alone.
