When you need it

The application itself — the letter you give to the office that issues Senior Citizen Cards, on plain paper, with your age and residence declared in the same document so that no separate affidavit is needed unless the office asks for one. For a person who wants a Senior Citizen Card in Punjab or Chandigarh to claim the concessions and facilities that senior citizens are given — where the office accepts a written application.

Which office issues the card, and how it is applied for (counter, Sewa Kendra, Sampark centre or an online portal), is for that office to say; ask it first and write its name in the first question. Not for a pension — use the Old Age / Widow Pension Affidavit. Not for a proof-of-age affidavit alone — use the Date of Birth Affidavit. Not for a general address proof — use the Residence Proof Affidavit.

What follows is written for Punjab and Chandigarh. It states the position there rather than describing how it varies, so the amounts below are the ones that apply — worth confirming at the sub-registrar’s office, because rates change. If the property is anywhere else in India, none of this is yours.

Stamp duty

The application is written on plain paper and carries no stamp duty. The declaration of age and residence is built into the application, so no stamp paper is needed unless the issuing office asks for a separate sworn affidavit, in which case the Date of Birth Affidavit or the Residence Proof Affidavit is used.

Registration

Not a registrable instrument. It is an application to the office that issues Senior Citizen Cards, which checks the age and residence against the papers enclosed. There is no Sub-Registrar step.

Notarisation

Not required in this form: the application and the declaration in it are signed by the applicant only. If the issuing office insists on a notarised or sworn declaration, have the declaration sworn on a separate affidavit — confirm with the specific office first.

Witnesses

None required.
