When you need it

You are applying for the Old Age Pension or the Widow Pension (also called Financial Assistance to a Destitute Woman) administered in Punjab by the Department of Social Security, Women and Child Development through the Sewa Kendra network, or in Chandigarh by the Social Welfare Department, Chandigarh Administration — and the application form asks for a sworn affidavit confirming your age, marital status, family income, residence, and that you are not already drawing a conflicting pension. Both schemes are welfare benefits, part-funded under the National Social Assistance Programme and topped up by the State or the Administration, sanctioned on age (or widowhood) and income criteria that are revised from time to time and checked again at renewal. This affidavit is filed with the first application and is commonly asked for again whenever the benefit comes up for renewal or review. Not for: a claim to a deceased government employee's family pension, which is a service-record matter for that employee's own department; or a claim to widow pension under the Employees' Pension Scheme, 1995, which the Employees' Provident Fund Organisation handles on its own form.

Stamp duty

An affidavit attracts a nominal duty under Article 4, Schedule 1-A to the Indian Stamp Act, 1899 as applicable to Punjab, which Schedule also governs the Union Territory of Chandigarh. In practice it is sworn on a non-judicial stamp paper of Rs. 10 — the smallest denomination generally sold at a treasury, an SHCIL e-stamp counter or an authorised stamp vendor in Punjab or Chandigarh — though several Social Security and Social Welfare counters accept this particular affidavit as a plain self-declaration with no stamp paper at all, precisely because the applicant is by definition elderly or widowed and the office would rather not send them back out to buy one. Ask the filing office before buying stamp paper. Figures checked 19 September 2026: Punjab's Schedule 1-A rates for a list of instruments including affidavits have been revised by a cabinet-approved ordinance since Article 4 was last generally known to charge one rupee, and the exact rupee figure now notified was not independently confirmed for this template — confirm it at the stamp vendor on the day of purchase.

Registration

This affidavit does not affect immovable property and is not an instrument the Registration Act, 1908 requires to be registered, in Punjab or in Chandigarh. It is not registered; the original goes into the pension file kept by the sanctioning office, and the Deponent should keep a signed and attested photocopy.

Notarisation

Sworn or affirmed before a Notary Public or an Oath Commissioner appointed for Punjab or for the Union Territory of Chandigarh, who verifies the Deponent's identity, administers the oath or affirmation, and signs, stamps and dates the affidavit. Where the filing office accepts a plain self-declaration for this particular benefit, as noted above, that step is skipped and the Deponent signs directly before the office's own official — confirm which route the filing office wants before the appointment is booked.

Witnesses

An affidavit is proved by the Notary's or Oath Commissioner's own attestation and needs no separate witness in either jurisdiction. Where the filing office's own application form calls for one or two identifying witnesses in addition — as some Sewa Kendra and Sampark Kendra counters do as a matter of local practice rather than law — follow that office's form.
