The Motor Vehicles Act, 1988 and the Central Motor Vehicle Rules, 1989 apply uniformly across India, and licensing is now largely routed through the Sarathi and Parivahan portals; what is jurisdiction-specific here is the authority you file with — in Punjab the Regional Transport Authority or the Sub-Divisional Magistrate-cum-Licensing Authority of the district, in Chandigarh the Registering and Licensing Authority, U.T. Chandigarh — and where this affidavit is notarised. Not for: a fresh learner's or permanent licence application by an adult with the usual documents, which needs no affidavit; or a driving licence dispute, suspension or disqualification, which is a hearing before the Licensing Authority or a court, not an affidavit.
