For an application to the Food, Civil Supplies and Consumer Affairs Department in Punjab, or the Food and Supplies Department in Chandigarh, for a new ration card, or to add a family member — on birth or on marriage into the family — or remove one — on death, on marriage out, or on the member permanently shifting to another household — from an existing card. This is the sworn declaration those departments ask an applicant to annex, confirming the family composition stated in the application and, importantly, that no member named is already covered under another ration card anywhere in India — the fact the National Food Security Act's one-card-per-family rule and Aadhaar seeding are both designed to catch. Filed through the Punjab Food Supply portal or a Suwidha Kendra, or Chandigarh's Sampark Kendra or e-District portal, alongside the department's own application form.
