कब ज़रूरत पड़ती है

Your name is spelled or ordered differently across your passport, your school/college marksheets or certificates, and an English test score report (IELTS, TOEFL, PTE, or similar) — and the visa authority or the admitting university wants a sworn affidavit confirming all versions refer to one and the same person before they will accept your application. More general name-reconciliation needs — not tied to a visa or an English test score — are better served by the Affidavit of One and the Same Person; use this version where a visa application or a university admission file is specifically what you are reconciling the name for.
