70 दस्तावेज़हलफ़नामा एक लिखित बयान है जिस पर आप दस्तख़त करके यह शपथ लेते हैं कि यह सच है — आमतौर पर नोटरी के सामने — ताकि कोई बैंक, सरकारी दफ़्तर, अदालत या कोई और व्यक्ति आपसे सीधे पूछे बिना उस पर भरोसा कर सके। किसी रिकॉर्ड में नाम या पता ठीक कराने, किसी प्रमाणपत्र की अर्ज़ी के साथ लगाने, बैंक या कर्ज़ के दावे के समर्थन में, या कोई ऐसी बात रिकॉर्ड पर रखने के लिए — जैसे किसी प्रॉपर्टी के असली वारिस कौन हैं — जो किसी और काग़ज़ में नहीं लिखी होती, हलफ़नामे की ज़रूरत पड़ती है।

नाम, पहचान और पता
इनमें से ज़्यादातर हलफ़नामे आपके अपने काग़ज़ों में अलग-अलग दिखने वाले नाम, स्पेलिंग या जन्मतिथि को ठीक करने के लिए हैं — शादी से पहले का नाम बनाम बाद का नाम, जन्म प्रमाणपत्र और आधार के बीच बदल गई स्पेलिंग, या किसी नाबालिग़ का नाम जिसके लिए माता-पिता को बच्चे की ओर से शपथ लेनी पड़ती है। कुछ इससे आगे जाकर नया नाम भारत के राजपत्र में छपवाने का काम करते हैं, या ऐसी जगह आपके चरित्र की पुष्टि करते हैं जहाँ दफ़्तर को काग़ज़ की जगह शपथ-पत्र चाहिए।
- Aadhaar Name/DOB Correction DeclarationYour name or date of birth is wrong on your Aadhaar card — a spelling error made at enrolment, a name that changed on marriage, or a date of birth recorded from an estimate rather than a document — and the Unique Identification Authority of India's own correction process, through the Self Service Update Portal, the myAadhaar portal, or an Aadhaar Seva Kendra, asks you to submit a document it accepts as proof, together with a signed declaration of what the correct particulars are and how the error arose. Aadhaar enrolment and update is governed by the Aadhaar (Targeted Delivery of Financial and Other Subsidies, Benefits and Services) Act, 2016 and the regulations made under it, which apply identically wherever in India the correction is made; what differs between Punjab and Chandigarh is only where this declaration is notarised, if the update centre you use asks for a notarised copy rather than a plain signed one. Not for: adding or correcting a mobile number or email address, which needs no supporting document and no declaration; correcting an address, which Aadhaar allows on a simple self-declaration without independent proof in defined circumstances; or a first Aadhaar enrolment, which is a different process entirely.₹199खोलें
- Affidavit of One and the Same PersonUse this when the same person's name appears differently across their papers — a maiden vs married name, an initial expanded in one paper but not another, a spelling/transliteration difference (common where a jamabandi was transliterated from Urdu or a regional script) — and an office needs assurance they're one person. Needed at: mutation, when a deed name doesn't match the jamabandi; registration; bank KYC; EPFO, LIC, passport, RTO; an electricity correction; a society; or an employer. It reconciles name versions — it does NOT change your name (a deed of change plus Gazette publication does) and does NOT correct Aadhaar/PAN. Nominal duty, notarised, not registered.₹199खोलें
- Date of Birth AffidavitA sworn declaration of a date of birth, for use where a birth certificate was never issued, cannot be traced, or the receiving office asks for a supporting affidavit alongside whatever other proof of age exists — school or college admission, a first employment record, a pension or provident fund claim, or an application where the birth was never registered with the municipal or panchayat authority at the time. Sworn by the person themself for their own date of birth, or by a parent or guardian for a minor's. - Gazette Name Change ApplicationFor an adult who has already sworn a name-change affidavit (see the separate Name Change Affidavit template in this library) and now wants the change formally published in the Gazette of India — the further step several authorities specifically insist on: government and public-sector service records, some professional council or registration bodies, and any counter that asks for a "public notice" of the change rather than the affidavit alone. This template bundles, in one filing: the covering application to the Department of Publication, the affidavit it must annex, and the draft text for the newspaper advertisement the Department's own instructions ask for alongside the Gazette entry. Filed through the Department's own e-Gazette name-change service at egazette.gov.in, or by post to the Department's office, whose current address that portal publishes. The Gazette procedure itself is a single central-government process common to every applicant in India; what this template localises is the applicant's own paperwork — the affidavit and the newspaper choice — for Punjab and Chandigarh. - Minor Name Change Affidavit (by Parent)A minor's name needs to be corrected or changed on record — a spelling error carried over from the birth certificate, a name never formally recorded at birth now being added, or a family's considered decision to change it — and, because the child cannot swear an affidavit themselves, a parent or guardian does it on the minor's behalf. Used to support a correction in the Register of Births under the Registration of Births and Deaths Act, 1969, a change in school records, or a passport application for the minor. This affidavit is ordinarily one of several papers such a correction needs, not the only one — the Registrar, school or passport office will commonly also ask for the original birth certificate and, sometimes, other supporting proof. - Name Change Affidavit (Adult)An adult wanting to formally change their name — on marriage, on religious conversion, to correct a spelling that has carried across documents, for personal preference, or for astrological or numerological reasons — needs a sworn affidavit before most authorities will update a record. This affidavit alone is what banks, employers, schools and universities commonly accept to update their own records in the new name. It is also Step 1 before the separate Gazette Name Change Application in this library, which some authorities — government service records, some professional registrations, a passport re-issue in certain categories — additionally require. - Name Change after Marriage AffidavitA spouse — most often, but not only, a wife adopting her husband's surname — who has changed their name because of marriage and needs a sworn affidavit linking the old name to the new one, with the marriage that caused the change, to update Aadhaar, PAN, a passport, bank accounts and KYC records, a driving licence, an employer's service records, or educational certificates. Unlike a bare name-change affidavit, this one recites the marriage itself — the spouse, the date, the place and whether it is registered — because that is exactly what UIDAI, a passport counter or a bank asks a post-marriage name change to show. Not for a name changed for any other reason — religious conversion, a spelling correction, or personal preference unconnected to marriage: use the Name Change Affidavit (Adult) in this library instead, which carries no marriage particulars. Not for a minor child's name, which a natural guardian affirms in different words using the Minor Name Change Affidavit. - Self-Declaration / Character AffidavitA general-purpose sworn declaration of good character and, where relevant, of criminal antecedents (or their absence) — asked for in job applications, admissions, licence applications (arms, liquor vend, private security and similar), tenancy verification, passport applications and a range of government forms across Punjab and Chandigarh that simply want "a self-declaration/character affidavit" annexed, without prescribing their own format. - BC/SC Certificate Supporting AffidavitFor an applicant seeking a Backward Class (BC) or Scheduled Caste (SC) certificate from the Tehsildar/SDM in Punjab or the Deputy Commissioner's office in Chandigarh, where the application asks for a sworn affidavit supporting the claimed caste, native place and — for a Backward Class claim only — that the family is outside the creamy layer. A Scheduled Caste claim carries no creamy layer exclusion: the creamy layer test applies only to Backward Classes/OBCs, following Indra Sawhney v. Union of India. This affidavit branches its declarations by which of the two is being claimed, so the creamy-layer questions appear only for a Backward Class claim. - Border Area Certificate Affidavit (Punjab)For an applicant from a village near the international border who is seeking a Border Area Certificate from the Tehsildar/SDM in Punjab — the certificate behind the Border Area Meritorious Candidates (BAMC) quota in admission to Punjab's medical, dental, engineering and certain other professional colleges, and behind Border Area Development Programme benefits. - EWS Certificate AffidavitFor a candidate applying for an Economically Weaker Sections (EWS) certificate — the income-and-asset self-declaration the Tehsildar or Sub-Divisional Magistrate's office in Punjab, or the Deputy Commissioner's office in Chandigarh, asks for before issuing the certificate that carries the 10% EWS reservation under the 103rd Constitutional Amendment, 2019, in admissions and public employment. The certificate itself is issued by the SDM/Tehsildar in Punjab, through the Suwidha Kendra or edistrict.punjabgovt.gov.in, or by the Deputy Commissioner's office in Chandigarh through Sampark; this affidavit is the sworn income-and-asset declaration that application asks the applicant to annex. EWS is available only to a family not already covered by any existing Scheduled Caste, Scheduled Tribe or Other Backward Classes (non-creamy-layer) reservation — confirm that before using this affidavit, since it is the wrong document for a candidate who already holds a caste certificate under one of those categories. - Family ID (PPP) Income/Member Correction Self-Declaration (Haryana)Your Family ID (Parivar Pehchan Patra) in Haryana shows the wrong annual income, is missing a family member, still lists someone who has died or moved out, or has a member's name, date of birth or relationship wrong — and the correction has to be requested through the Family ID portal or a Common Service Centre, together with a signed self-declaration of what the record should say and a supporting document. The Family ID / Parivar Pehchan Patra is a scheme of the Government of Haryana, administered by the Citizen Resources Information Department, and does not exist in Punjab or in Chandigarh, so this document is drafted for Haryana specifically rather than for the two jurisdictions the rest of this library serves. - Rural Area Certificate Affidavit (Punjab)For an applicant seeking a Rural Area Certificate from the Tehsildar/SDM in Punjab, certifying ordinary residence in a rural, non-municipal area of the State — asked for by the rural-area weightage some Punjab professional college admissions (agriculture, veterinary and certain other courses) and some state recruitment and scheme notifications give to rural candidates, and as supporting proof for agricultural-scheme eligibility. "Rural" here means outside every Municipal Corporation, Municipal Council and Nagar Panchayat, and outside any area otherwise notified as urban — the same urban/rural line drawn in Punjab's own revenue and local-government notifications. - Arms Licence Affidavit (Punjab/Haryana)You are applying for a fresh arms licence, its renewal, the addition of another firearm to an existing licence, or a duplicate licence in place of one lost or damaged, under the Arms Act, 1959 and the Arms Rules, 2016, and the Licensing Authority's own application form asks for a sworn affidavit of your antecedents, fitness and purpose alongside it. The Arms Act, 1959 and the Arms Rules, 2016 apply uniformly across India; the Licensing Authority for this affidavit is the District Magistrate or Deputy Commissioner of the district in Punjab, or the Licensing Authority notified for the Union Territory of Chandigarh, and it is that office's own local verification — through the police station concerned — that the rest of the application turns on. Not for: a licence for a firearm used in film or theatre production, or an arms dealer's stock-in-trade beyond what a dealer's own licence covers, each of which follows a separate procedure under the Rules; or a permit to carry the weapon into another state, which is a separate authorisation.₹199खोलें
- Driving Licence AffidavitYou need a sworn affidavit to support a driving licence matter the Regional Transport Authority or Licensing Authority has no independent proof for — a duplicate licence after the original was lost or destroyed, your date of birth where no birth or matriculation certificate exists, a change of the address or the name recorded on your licence, or a parent's consent to a minor's learner's licence for a gearless motorcycle. The Motor Vehicles Act, 1988 and the Central Motor Vehicle Rules, 1989 apply uniformly across India, and licensing is now largely routed through the Sarathi and Parivahan portals; what is jurisdiction-specific here is the authority you file with — in Punjab the Regional Transport Authority or the Sub-Divisional Magistrate-cum-Licensing Authority of the district, in Chandigarh the Registering and Licensing Authority, U.T. Chandigarh — and where this affidavit is notarised. Not for: a fresh learner's or permanent licence application by an adult with the usual documents, which needs no affidavit; or a driving licence dispute, suspension or disqualification, which is a hearing before the Licensing Authority or a court, not an affidavit.₹199खोलें
- Electricity Connection Transfer AffidavitA domestic, commercial or industrial electricity connection has to move into a new name — because the person it stood in has died, because the premises it serves have been sold or let to someone else, or because another member of the same household is taking it over — and the distribution licensee (PSPCL in Punjab, or the department supplying electricity to Chandigarh) asks for an affidavit alongside its own name-transfer or mutation-of-connection form before it re-registers the account. The library also carries the Application for Transfer of Electricity Connection (Change of Name), which is the formal application used where the transfer follows a property mutation. This affidavit is the general sworn declaration a distribution office asks for alongside that application, or on its own for a same-household takeover that involves no mutation. - Gas Connection Transfer AffidavitA domestic LPG or piped natural gas connection has to move into a new name — because the person it stood in has died, because the premises it serves have been sold or let to someone else, or because another member of the same household is taking it over — and the distributor (an Indian Oil, HP Gas or Bharat Gas agency, or a piped-gas utility) asks for an affidavit alongside its own transfer form before it re-registers the connection. The distributor's own transfer procedure follows the marketing discipline guidelines its oil marketing company issues, not a statute specific to Punjab or to Chandigarh, so what is jurisdiction-specific here is chiefly where this affidavit is notarised and on what value of stamp paper. Not for: a first application for a new gas connection, which is a different form; or a complaint about a distributor's refusal to transfer, which is taken up with the distributor and, if unresolved, with the oil marketing company's consumer grievance cell.₹199खोलें
- Water/Sewer Connection AffidavitYou are applying for a fresh water and sewerage connection, or continuing an existing one in your own name after buying, inheriting or renting the property, and the Municipal Corporation, Municipal Council or Chandigarh's Engineering / Public Health Wing asks for a sworn affidavit of ownership or occupancy alongside its own connection application form, to confirm who is asking for the connection and that the same property is not drawing a second, undeclared one. In Punjab this sits under the Punjab Municipal Corporation Act, 1976 for a Municipal Corporation area, the Punjab Municipal Act, 1911 for a Municipal Council or Nagar Panchayat area, and the Punjab Water Supply and Sewerage Board Act, 2018 where the Board itself operates the network; in Chandigarh the Municipal Corporation of Chandigarh runs the network under the Punjab Municipal Corporation Act, 1976 as extended to the Union Territory. Not for: a complaint about a leaking pipe, low pressure or a blocked sewer at an already-connected property, which is a service request and not an affidavit; or a request to disconnect.₹199खोलें

- Old Age / Widow Pension AffidavitYou are applying for the Old Age Pension or the Widow Pension (also called Financial Assistance to a Destitute Woman) administered in Punjab by the Department of Social Security, Women and Child Development through the Sewa Kendra network, or in Chandigarh by the Social Welfare Department, Chandigarh Administration — and the application form asks for a sworn affidavit confirming your age, marital status, family income, residence, and that you are not already drawing a conflicting pension. Both schemes are welfare benefits, part-funded under the National Social Assistance Programme and topped up by the State or the Administration, sanctioned on age (or widowhood) and income criteria that are revised from time to time and checked again at renewal. This affidavit is filed with the first application and is commonly asked for again whenever the benefit comes up for renewal or review. Not for: a claim to a deceased government employee's family pension, which is a service-record matter for that employee's own department; or a claim to widow pension under the Employees' Pension Scheme, 1995, which the Employees' Provident Fund Organisation handles on its own form.₹199खोलें
- Ration Card AffidavitFor an application to the Food, Civil Supplies and Consumer Affairs Department in Punjab, or the Food and Supplies Department in Chandigarh, for a new ration card, or to add a family member — on birth or on marriage into the family — or remove one — on death, on marriage out, or on the member permanently shifting to another household — from an existing card. This is the sworn declaration those departments ask an applicant to annex, confirming the family composition stated in the application and, importantly, that no member named is already covered under another ration card anywhere in India — the fact the National Food Security Act's one-card-per-family rule and Aadhaar seeding are both designed to catch. - General Indemnity BondSomeone — a bank, a company, a society, an authority or another private party — will only act, release something or accept a risk if you first promise in writing to make good any loss that decision causes them: releasing a fixed deposit without the original receipt, issuing a duplicate certificate, letting you proceed without a document they would normally insist on, or another one-off situation not already covered by a ready-made document elsewhere in this library. This is a general-purpose bond for exactly that situation — state what you are asking the Obligee to do or accept, and what you are indemnifying them against, and the Bond does the rest. It is a personal promise to pay, not a mortgage or a charge on any property, and it does not by itself make the Obligee do what Recital A describes; that agreement is separate, and this Bond only backs it. - Court Marriage Affidavit (Age and Marital Status)Each person intending a civil ("court") marriage under the Special Marriage Act, 1954 gives this affidavit — one per party, not one for the couple — alongside the notice of intended marriage under section 5 of that Act, to the Marriage Officer of the district where at least one of them has resided for the thirty days immediately before the notice. It declares age, present marital status, mental capacity to consent, and that the two are not within a degree of relationship the Act prohibits unless a custom governing them permits it. Not the notice of intended marriage itself, which the Act prescribes its own form for, and not the declaration the parties and three witnesses sign at the marriage under section 11 of that Act — this affidavit supports the notice stage only. Different Marriage Officers ask for it in slightly different forms in practice, so check the specific office's own requirement before the appointment. - Marriage AffidavitA married person who needs a general sworn statement confirming their marriage — its date, place and the fact that it took place — for a bank or joint account KYC update, an employer's personnel records, adding a spouse as an insurance nominee or dependant, a child's school or college admission, a housing society or utility record, or any other counter that asks for proof of marriage and will accept a notarised affidavit rather than, or in addition to, a marriage certificate. Since 2025, Estate Office auto-mutation triggers digitally the moment the deed registers — there's no separate mutation application — so this affidavit-cum-indemnity bond must be in the file WHEN THE DEED IS PRESENTED; there's no later counter to file it at. Missing or defective, you get a registered title but a stalled mutation with no application to chase. Two form answers decide if this is even the right paper: the record track (Estate Office sectors, vs a village/lal dora/phirni area, which uses the revenue track instead) and tenure (freehold vs leasehold — leasehold needs the lease particulars, ground rent, NOC and one-third unearned-increase entries filled in too). Annexure-II, an architect's completion/habitability certificate, is needed wherever a building stands. Don't use this: for Punjab property (intkal through the Patwari/Kanungo/CRO is a different template); for a village/lal-dora/phirni-area property; for a mere name addition/deletion of a relative under Rule 7(iii) (not a transfer); or for a residential transfer outside the four categories the Estate Officer's 10 February 2023 freeze still permits (unresolved whether that freeze has lifted). - Affidavit and Indemnity Bond for a Lost Title DeedYou need this when someone won't act without the original registered deed: a bank/HFC won't sanction a loan or create an equitable mortgage without it; a buyer's advocate has flagged it missing in a title search; a society or development authority won't transfer records; the Tehsildar's mutation (intkal) is stuck; or the discom won't transfer the connection. FIRST CHECK WHO LOST IT. If a bank/HFC held the original under an equitable mortgage and lost it — common once a loan is closed — the LENDER should swear the loss, give the indemnity, and file its own police report. Don't sign an affidavit for a loss that isn't yours; get the lender's loss letter first. USUAL SEQUENCE: (1) police loss report; (2) certified copy (nakal) of the deed from the Sub-Registrar under s.57, Registration Act, 1908; (3) public notice in one English and one regional newspaper inviting objections; (4) a fresh Encumbrance Certificate; (5) then this affidavit plus indemnity. FIR OR LOST REPORT. Simply losing a document isn't a cognizable offence, so police normally record a Lost Article Report/NCR under s.174, BNSS 2023 (was s.155 CrPC) — that's what banks want. Theft is different: an FIR under s.173 BNSS (was s.154 CrPC) applies there. WHAT A CERTIFIED COPY DOES NOT DO. Under s.57(5) it proves the deed's contents and satisfies secondary-evidence law (s.65, Evidence Act 1872 / Bharatiya Sakshya Adhiniyam 2023) — but it isn't a substitute original. A mortgage by deposit of title deeds (s.58(f), Transfer of Property Act, 1882) needs the physical document; there's no such thing as a "duplicate deed." - Spelling Correction Affidavit for a Property RecordA record has a spelling variation, and the authority wants a sworn identity statement with the correction application. Narrow scope: the two spellings, documents showing both refer to the same person, and that no ownership/share transfers. - Liability Affidavit-cum-Indemnity Bond (Estate Office Annexure-2)For the transferee taking a Chandigarh sector property — buyer, donee, or party under a blood-relations transfer, exchange, or family settlement — swearing this before the Estate Office mutates it into your name. Every transferee in every transaction files it (Annexure-2), sworn by the person coming IN. DON'T USE YET if the property is RESIDENTIAL and the transfer doesn't leave ONE person or family holding the whole unit. Since the Supreme Court's 10 Jan 2023 judgment, the Estate Officer's Public Notice (10 Feb 2023) permits transfer only where one person/family gets the whole unit, it's within the family, or all co-owners after are one family — otherwise transfers are STOPPED pending the Heritage Conservation Committee (status unconfirmed): ask the Estate Office BEFORE buying stamp paper. This affidavit can't revive a stopped mutation. Timing is critical: auto-mutation (since 2025) sends the deed digitally to the Estate Office on registration, with NO separate application after — so this must be sworn, stamped, and IN the file BEFORE the deed is presented. Six sworn facts: no court attachment/injunction; no pending cancellation/resumption; no building violation/misuse; nothing owed (premium, ground rent, misuse charge, water/property tax); the storey actually complete; and your undertaking to finish construction, get the Occupation Certificate, and execute the Redemption Deed. Each box reads 'save as disclosed' — a BLANK box is a denial on oath (clause 1.5), and the Estate Office can check three of four against its own portal in minutes. If every recorded co-sharer is selling and each signs the application and the sale papers personally, this separate affidavit is usually unnecessary — it exists for the case where some co-sharers stay out of the transaction but the Collector still needs their concurrence on file. - Shop & Establishment AffidavitYou are registering a shop, office or commercial establishment under the Punjab Shops and Commercial Establishments Act, 1958 — as applicable in the State of Punjab, and as extended to and applied in the Union Territory of Chandigarh — and the local authority or the online portal wants a sworn affidavit from the proprietor, partner or authorised signatory confirming the establishment's particulars before it issues the registration certificate. Many municipal bodies now accept a self-certified online declaration for a first registration, without a notarised affidavit; use this document where a sworn affidavit is specifically asked for — on renewal, on a change of particulars, where the online route is unavailable for the applicant's category of establishment, or simply because the office handling the application wants one on file. It affirms who runs the establishment, what it does, where, since when, how many people work there, and its weekly holiday and working hours, and undertakes to comply with the Act. - Proprietorship DeclarationA bank, GST authority, government department or tender wants proof that a business is a sole proprietorship — one individual owns it, with no partner, no company and no separate legal existence apart from that individual — and wants it in the form of a signed and sworn declaration rather than, or in addition to, other registration certificates. The Reserve Bank of India's KYC directions for opening a current account in the name of a proprietary concern normally call for any two of a list of documents showing the firm's name and address — a registration certificate, GST certificate, Udyam certificate, importer-exporter code, a licence issued by a municipal authority, or similar — and, where the bank cannot get two such documents, a declaration from the proprietor confirming sole proprietorship, together with any other document the bank reasonably asks for. This document is that declaration, drafted as a sworn affidavit so it is accepted wherever a plain declaration would not be. For an individual who is the sole proprietor of a business in Punjab or Chandigarh, wanting to prove that status to a bank, authority or other third party. - Trade Licence AffidavitYou are applying to a Municipal Corporation or Municipal Council for a trade licence to run a trade the municipal law treats as one requiring a licence — commonly the "dangerous and offensive" trades scheduled under the applicable municipal Act, such as an eatery or dhaba using fire, a business storing or selling chemicals, timber, scrap, or similar goods, or another trade the local byelaws specify — and the municipal office wants a sworn affidavit from the applicant along with the application form. In Punjab this is issued under the Punjab Municipal Corporation Act, 1976 in a Corporation area, or the Punjab Municipal Act, 1911 in a Municipal Council or Nagar Panchayat area; in Chandigarh, under the Punjab Municipal Corporation Act, 1976 as extended to and applied in the Union Territory, administered by the Municipal Corporation, Chandigarh. The affidavit affirms the trade carried on, the premises it is carried on at, whether the applicant owns or occupies the premises with the owner's consent, and any fire or pollution clearance the trade needs before a licence can issue. For a trade licence application or renewal in Punjab or Chandigarh. - Affidavit of Legal Heirs for a Virasat Intkal (Inheritance Mutation)The annexure affidavit for a virasat intkal — when someone who owned Punjab village land dies, the Halqa Patwari enters the mutation and the Circle Revenue Officer sanctions it, reading this with the shajra nasab and death proof; without it the file sits. Also accepted by banks, insurers, PSPCL, societies, employers and depositories as heir proof — evidence only, at the acceptor's risk. Do NOT use if: a will exists (probate governs); an heir gives up a share (becomes a chargeable, registrable Release — no mutation on it; partition is separate, AFTER the intkal); heirship is disputed or a suit/appeal is pending; or the fix is to an entry already IN the jamabandi (a fard badar, not a mutation). - Delayed Death Registration AffidavitA death was not registered within the twenty-one days the Registration of Births and Deaths Act, 1969 ordinarily allows, and a delayed registration application needs a sworn affidavit to go with it — the Act itself requires one once the delay passes thirty days. Between thirty days and one year after the death, this affidavit accompanies an application for the Registrar's written permission to register it, together with the prescribed fee. Beyond one year, it accompanies an application to a Magistrate of the First Class, who alone can order registration at that point, after verifying the death. Confirm which of the two bands applies to your dates before filling this in, and expect the Registrar or the Magistrate to ask for independent proof of death — a hospital record, a doctor's certificate, a cremation or burial receipt — alongside this affidavit, not in place of it. - Indemnity Bond for Deceased's Bank ClaimAn account holder has died, no nominee covers the balance, and the legal heirs want the bank to release it without going to court first for a succession certificate, letters of administration or probate. Most banks allow this below a threshold fixed by their own board-approved policy — sometimes with a surety, sometimes without one — in exchange for an indemnity bond from the claiming heir(s): a promise to make the bank good if somebody else later turns out to have a better right to the money. This bond does not decide who the legal heirs actually are — it assumes that question is settled, by a legal heir certificate or otherwise, and simply protects the bank for paying out on that basis without a court order. Where heirship itself is disputed, a bank will commonly decline this route regardless of what this bond says. - No-Claim Affidavit by an HeirAn authority, buyer or lender asks an heir for this so a transaction can proceed — evidentiary only: a limited no-objection, disclosing any settlement, NOT transferring a vested share. - RTI Second Appeal or Complaint to the Information CommissionThis is the last step inside the RTI system. File it when the first appeal has been decided against you, or has not been decided within thirty days of its receipt, or forty-five where the appellate officer recorded reasons for more time. The second appeal goes to an Information Commission, not to the public authority: to the Punjab State Information Commission, at the Red Cross Building in Sector 16, Chandigarh, where the public authority is under the Government of Punjab, and to the Central Information Commission where it is an office of the Union Territory of Chandigarh Administration. It must be filed within ninety days from the date the first appeal's decision was received or should have been made, and the Commission can admit a later appeal for sufficient cause. The Commission takes an appeal up only after the first appeal is decided or forty-five days have passed since it was filed. The same form serves as a complaint under section 18 where no Public Information Officer would take your application, the fee demanded is unreasonable, or the information given is incomplete, misleading or false. Attach self-attested copies of the application, the reply, the first appeal and the order, with an index, and verify the facts at the foot; the Punjab Commission also wants at least three sets in paper-book form for an appeal, a statement that the matter has not been filed before and a self-attested identity document. The Commission can order the information to be supplied, require compensation for loss you suffered, and fine the officer two hundred and fifty rupees a day, up to twenty-five thousand rupees.₹199खोलें
