When you need it

You have lost documents, a mobile phone, a passport or another article in Punjab or Chandigarh, and an office, bank or telephone company has asked for a police report before it issues a duplicate or blocks the article. This report tells the police station what was lost, when and where, how you think it was lost, what you have done to find it and why you need the report.

It asks the station to enter the loss in its records and give you an acknowledgement or report number. It is not an FIR and alleges no offence. If you believe the article was stolen, say so in the report, or use the Police Complaint (General) if you were cheated or threatened. Not for a lost property document that needs a sworn statement: for that, use the Affidavit and Indemnity Bond for a Lost Title Deed or the Lost Document Affidavit (Passport / Share Certificate), which many offices ask for as well. A police report does not by itself give you a duplicate; the issuing office decides what else it needs.

What follows is written for Punjab and Chandigarh. It states the position there rather than describing how it varies, so the amounts below are the ones that apply — worth confirming at the sub-registrar’s office, because rates change. If the property is anywhere else in India, none of this is yours.

Stamp duty

Punjab and Chandigarh: no stamp duty is payable. The report is a letter to an authority; it creates, transfers or records no right in any property, and it is signed by the reporter alone.

Registration

Punjab and Chandigarh: not registrable. It is a letter to an authority and deals with no right in immovable property, so section 17(1)(b) of the Registration Act, 1908 does not reach it.

Notarisation

Punjab and Chandigarh: no notary is needed. The reporter signs the report. A sworn affidavit of loss is a separate paper that some offices ask for in addition; see the Lost Document Affidavit.

Witnesses

Punjab and Chandigarh: no witness signs the report. Anyone who saw the article being lost, or who has found it, may be named in the report with a telephone number.

LOST ARTICLE REPORT

To: The Officer in Charge, Police Station Division No. 5, Ludhiana

Date: 2 April 2026

From: Manjit Kaur, wife of Balbir Singh, aged 58, of House No. 221, Street No. 4, Gurdev Nagar, Ludhiana 141001, telephone +91 98723 11418, email manjit.kaur221@example.com

Subject: Report of the loss of my passport and driving licence

Sir or Madam,

  1. 1. What I have lost. I have lost the document or documents described below: Indian passport No. M1234567 issued at Chandigarh, and Punjab driving licence No. PB10 2019 0012345 issued at Ludhiana. For the phone, the number and IMEI details are number 98723 11418, IMEI 356938035643809.
  1. 2. When and where. I lost it on or about 1 April 2026, at or near the Clock Tower bus stand, Ludhiana. I last had it when I got on the bus at about 6 p.m. and noticed it was missing when I reached home at about 8 p.m.
  1. 3. What I have done to find it. I searched the bus and the bus stand, asked the conductor, and rang my phone several times; it was switched off.
  1. 4. Lost or stolen. I believe the article was lost and not stolen.
  1. 5. Misuse. I am worried about misuse of my SIM card and the UPI apps on the phone. I will inform the police at once if I learn that the article, or anything in it, is being misused.
  1. 6. Why I make this report. I make this report to apply for a duplicate passport and driving licence and to have my SIM blocked.
  1. 7. My request. Please enter this report in the station's records and give me an acknowledgement or report number that I can show to the office that asks for it. I hand this report in writing at the police station.
  1. 8. If I find it, and the truth of this report. If I find the article, I will tell this police station and the office to which I have given this report. What I have stated is true to my own knowledge, and I have left out nothing I know about the loss. Enclosed: a copy of the phone bill.

LEGAL BASIS

Punjab: A report of a lost article is a record kept at the police station within whose area the article was lost. It alleges no offence and is not a first information report; if the article was stolen, the police are told so and treat the matter as a complaint of theft instead. Offices that issue duplicates, such as a passport office or a licensing authority, commonly ask for the police report, but each decides for itself what it requires.

Chandigarh: The same applies in the Union Territory: the report is made at the police station in whose area the article was lost, alleges no offence, and is not a first information report. The office issuing the duplicate decides what it requires.

PERSONAL DATA NOTICE

This notice is given under the Digital Personal Data Protection Act, 2023.

The purpose of collecting the personal data in this document is to identify the reporter and the lost article, and to record the loss, so that the police can enter the report and the reporter can obtain a duplicate or block the article.
