You want a school, a board or a university to correct your name or date of birth in your certificate, marksheet or admission record, and the institution asks for a sworn affidavit with your correction application. This is that affidavit.

It states what the record shows, what it should show, how the difference arose and which papers prove the correct entry. Not for a date of birth where no birth certificate exists at all (use the Date of Birth Affidavit). Not for a name that differs between a passport and a marksheet (use the Name Mismatch Affidavit). Not for a legal change of name (use the Gazette Name Change Application). This affidavit does not change the record; the institution decides the correction under its own rules and time limits.

AFFIDAVIT FOR CORRECTION OF NAME OR DATE OF BIRTH IN SCHOOL OR UNIVERSITY RECORDS

I, Gurpreet Kaur, D/o Sh. Jaswant Singh, resident of House No. 214, Sector 40-C, Chandigarh, am the person whose record is to be corrected, and do hereby solemnly affirm and declare as under:

1. That the record concerned is the B.A. degree and marksheets of 2019 (Roll/Registration/Enrolment No. PU-2016-04512) held by Panjab University, Chandigarh, and it is the record of Gurpreet Kaur.

2. That the entry for name in that record reads: Gurpreet Kour; 12 March 1998, whereas the correct entry is: Gurpreet Kaur; 12 March 1997.

3. That the difference arose as follows: The spelling was entered wrongly from the school form at the time of admission

4. That the correct entry is supported by the following documents, copies of which are enclosed with the application: Birth certificate no. 4521 of Municipal Corporation Chandigarh; Class 10 certificate

5. That the person named in paragraph 1 and the person shown in the record are one and the same person.

6. That no court order about this correction exists, and no other application for it is pending before any court or authority.

7. That I make this affidavit in support of the application to Panjab University, Chandigarh for correction of the entry, to be dealt with under that institution's own rules and procedure, and I understand that this affidavit does not itself change any record.

VERIFICATION

Verified at Chandigarh on 1 April 2026 that the contents of paragraphs 1 to 7 above are true and correct to my knowledge and belief, that nothing material has been concealed, and that no part of it is false.
