Gazette Name Change Application

When you need it

For an adult who has already sworn a name-change affidavit (see the separate Name Change Affidavit template in this library) and now wants the change formally published in the Gazette of India — the further step several authorities specifically insist on: government and public-sector service records, some professional council or registration bodies, and any counter that asks for a "public notice" of the change rather than the affidavit alone. This template bundles, in one filing: the covering application to the Department of Publication, the affidavit it must annex, and the draft text for the newspaper advertisement the Department's own instructions ask for alongside the Gazette entry. Filed through the Department's own e-Gazette name-change service at egazette.gov.in, or by post to the Department's office, whose current address that portal publishes. The Gazette procedure itself is a single central-government process common to every applicant in India; what this template localises is the applicant's own paperwork — the affidavit and the newspaper choice — for Punjab and Chandigarh. Not for a company, trust or firm changing its name — that goes through the Registrar of Companies or Firms, not this route.
