When you need it

Use this when your mobile, landline or broadband provider has left you without a service you pay for, charged you wrongly, or not acted on a complaint, and you want to put it in writing. The same form serves two steps: choose "first complaint" to write to the provider's complaint or nodal officer, or choose "appeal" to write to its appellate authority against a decision on your earlier complaint; the rules of the telecom regulator set up both steps and the times for each, which you should confirm.

Quote the complaint (docket) number the provider gave you. Answer from your bills and your service messages; never write a PIN, OTP or password, and give only the last four digits of an account or a card. Send the signed letter and keep the proof. If the provider does not put it right, the complaint can go to the District Consumer Commission through the Consumer Complaint to the District Commission, since telecom is a service under the consumer law. Not for a broadband or electricity connection a landlord must allow in a tenancy: see the landlord consent documents.

What follows is written for Punjab and Chandigarh. It states the position there rather than describing how it varies, so the amounts below are the ones that apply — worth confirming at the sub-registrar’s office, because rates change. If the property is anywhere else in India, none of this is yours.

Stamp duty

Punjab and Chandigarh: no stamp duty is payable. A letter of complaint creates, transfers or extinguishes no right, so it is not an instrument within section 2(14) of the Indian Stamp Act, 1899, and you sign it alone.

Registration

Punjab and Chandigarh: not registrable. This letter creates or transfers no right in immovable property, so section 17(1)(b) of the Registration Act, 1908 does not reach it. Keep the proof of sending, the provider's docket number and your bill.

Notarisation

Punjab and Chandigarh: no notarisation is needed. The letter works because it is in writing, dated and reaches the other side, not because anyone attests your signature.

Witnesses

Punjab and Chandigarh: no witness is needed. You sign alone.

TELECOM OR BROADBAND COMPLAINT AND APPEAL

Complaint or appeal about a telecom or broadband service

Date: 1 April 2026, at Ludhiana

I am sending this by registered post with acknowledgement due. I am also emailing a copy to appellate@exampletelecom.example.com.

To: The Appellate Authority, Example Telecom Ltd, Punjab Circle, Example Telecom House, Sector 34, Chandigarh 160022

From: Gurpreet Singh Dhillon, House No. 88, Urban Estate Phase 2, Patiala 147002 (email gurpreet.dhillon@example.com, telephone +91 98150 47731)

Subject: No broadband service since 3 November 2026 on connection ending 6120; complaint docket 90412266

Sir or Madam,

  1. 1. What this is. This is my complaint to you, made for the first time about this matter. The complaint (docket) number is 90412266 dated 4 November 2026.
  1. 2. My service. This concerns my fibre broadband connection ending 6120 at the address above, for which I am the subscriber.
  1. 3. The problem. The connection has been down since 3 November 2026. Technicians were promised on 4, 6 and 9 November 2026 but did not come. I have still been billed for the month. The bill amount I dispute is ₹1,180 (Rupees One Thousand One Hundred and Eighty only).
  1. 4. What I have done so far. I called the helpline on 4, 6 and 9 November 2026. On 12 November 2026 the complaint was closed as "resolved" though the service had not been restored.
  1. 5. What I require. Restore the service within three days, withdraw the bill for the days it was down, and compensate me for the loss of service. Please also tell me in writing, within the time the telecom regulator's rules allow, what you have done and the name of the officer handling this.
  1. 6. If it is not put right. If the matter is not put right, I will take it further to the appellate authority or the telecom regulator as the rules allow, and I may file a consumer complaint before the District Consumer Commission and ask for compensation, interest and costs. This letter does not waive any of my rights.

ENCLOSURES

  1. 1. Bills for October and November 2026. 2. Messages and call log showing my complaints. 3. The provider's closure message of 12 November 2026.

LEGAL BASIS

Punjab and Chandigarh: The telecom regulator's rules on the redressal of telecom consumers' complaints require a service provider to run a complaint centre and nodal officers, to give a docket number, to dispose of a complaint within a stated time, and to have an appellate authority to which a subscriber may appeal; a subscriber who is still not satisfied can complain to the regulator and to the consumer forum. A telecom provider's service to its subscriber is a service under the Consumer Protection Act, 2019, so a complaint of deficiency can be taken to the District Commission within two years of the cause of action. No fee is payable on this letter.
